Dr. Nichols is also a notable physician in the emerging field of endocrine disrupting chemicals. He is board certified in physical medicine and rehabilitation and is well known for his skill and expertise in the areas of interventional spinal procedures. J. Keith Nichols, M.D. is the Medical Director and CEO of Tier 1 Health and Wellness, a preventive medicine clinic headquartered the Chatanooga, TN. He and his wife, Angie founded Tier 1, a preventative medicine program dedicated to promoting optimal mental, physical and sexual health throughout the aging process. Dr. Nichols is also author and a highly revered international speaker.
I am originally from Georgia and received my medical degree from the
Medical College of Georgia in Augusta, Georgia. Following an internship at
Spartanburg Regional Medical Center in Spartanburg, South Carolina, I
completed my residency in Physical Medicine and Rehabilitation at the
University of Louisville in Louisville, Kentucky.
Over the last several years, I have traveled the country to gain education and
insight from the most well-respected and knowledgeable leaders in the
medical speciality of Bioidentical Hormone Replacement Therapy. I have
completed the rigorous process and joined an elite group of practitioners in
becoming one of a few certified in Advanced Bioidentical Hormone
Replacement Therapy in order to provide my patients with the most
comprehensive care available.
I have always maintained an active lifestyle, and my fitness regimen includes
activities such as mixed martial arts, weightlifting and mountain biking
combined with a healthy diet. Recently I began noticing a decrease in
strength, endurance and energy and began having nagging aches and pains
that did not respond to conventional medical treatment. I was experiencing
extreme fatigue, and after obtaining lab work and undergoing a physical exam
by my personal physician, I was told that my symptoms were “normal and
that I was just getting old.”
I began gaining weight, became increasingly irritable, and my quality of sleep
was terrible. I would start each day with plans to reverse this pattern, but by
late afternoon I was exhausted and had no desire for physical activity. I found
myself rapidly deteriorating and in a downward physical and mental spiral.
Through my own testing, I was found to have low testosterone, DHEA and
thyroid hormones. After replenishing all of my hormones back to youthful
levels, the deterioration subsided. I regained my energy, endurance and
muscle strength. I began losing weight, and developed a normal sleep
pattern. I noticed an increase in mental alertness, was no longer irritable, and
best of all, the nagging aches and pain had disappeared. In general, I felt
better than I had in years.
Through my own experience with low hormone levels, I found that my
situation was common to the patients I treat on a daily basis as a part of my
spine and sports medicine practice. These patients need to maintain their
activity level to prevent or treat their various musculoskeletal injuries, but they don’t have the energy, strength, motivation or physical ability to do so. It is my opinion that these symptoms are secondary to hormone imbalance and can be improved or reversed with bioidentical hormone supplementation.

My wife, Angie, and I maintain a healthy lifestyle and enjoy various forms of exercise to maintain health and fitness. I enjoy spending time at the lake with our friends and family. My other hobbies include mountain biking, mixed martial arts, and shooting sports (pistol and long-range marksmanship).
